When a survey is filled out by a contact, the answers can be stored in a profile extension linked to the audience list this is an option to be set in the form properties). This is a 1:1 profile extension if the contact can only participate once in the form, or a 1:n relation if they can participate multiple times.
For each question component in the form a field is created in the form list when performing an update of the datastore. The answer given by the contact is stored as such in the question field.
For matrix questions, the data is stored as following: 3_3|2_5. Where the text before the underscore (_) is the selected row in the matrix and the text after the underscore the selected column.
The list also stores information on when the form was modified and if it is completed. When the COMPLETED field is set to 1, the form was completed by the contact.
